If someone intentionally infects another with a potentially deadly disease from which the victim ultimately dies, assuming he was forewarned and it was witnessed by valid witnesses, would a Sanhedrin in the time of capital punishment execute the perpetrator or would the death be considered insufficiently direct?

Would a goel hadam (redeemer of the blood) be allowed to execute the perpetrator, without a ruling of the court; and/or even without actual warning/intention of the perpetrator?

What if, for example, the perpetrator infected an intermediary who (inevitably) infected the victim, e.g. a caregiver of a vulnerable patient?
