What is the source of the additions before תתקבל צלותהון, יהא שלמא, עושה שלום in Kaddish Tiskabel?
In Kaddish Tiskabel, there are parts to be said (see page 131) by the congregation just before the three phrases that begin: תתקבל צלותהון, יהא שלמא, עושה שלום.

Why do we comfort a mourner with Hamakom Yenachem Eschem?
Why, when comforting a mourner at the completion of a Shiva visit, do we refer to G-d as Hamakom? We say Hamakom Yanachem Eschem - why not Hashem Yenachem or Elokim Yenachem?
